This program provides healthcare services to qualified, adult indigent residents of Garza County. Proof of income, proof of address, proof of ID and resources are required to determine eligibility. You may contact the office for an application or you may use the link below to fill out the application. It will then need to be printed and mailed to the Garza County Healthcare District at P.O. Box 206 Post Texas 79356 or delivered in person 1002 North Avenue S.

Once a completed application is received along with all supporting documentation, a decision regarding your eligibility​ will be made within 14 business days. If you have any questions, you may contact our office during normal business hours.
